Congenital left ventricular diverticula are rare cardiac malformations. In our case a diverticular apical lesion of the left ventricle was accidentally found on a thoracic CT scan, acutely performed to rule out abdominal aortic rupture.
Left ventricular protrusion | Congenital left ventricular aneurysm | Clefts and crypts | Diverticulum | Cardiac hernia |
---|---|---|---|---|
Causes | Abnormality during embryogenesis Isolated congenital defect | Failure to resorb the trabeculated wall during embryological development phase | Congenital malformation(Isolated or in association with others congenital midline thoraco-abdominal defects and other malformations) Cantrell’s syndrome | Congenital pericardial defect Cantrell’s syndrome Chest wall trauma Surgery (ex.pericardial window) |
Morphology | Thin walled Pericardium Wide communication Paradoxical expansion in systole (a-dyskinesia) | Narrow deep invaginations within the myocardium localised more often in basal posterior septum or inside left ventricle free wall | Thick walled Pericardium Narrow communication Can be divided in contracting or non-contracting | Thickness can vary (strangulation) Pericardial defect (intermediate size) |
Histology | Fibroelastic endocardium Trabeculae and chordae tendineae | Crossing and interdigitation of myocytes that may predispose to myocardial splitting | Muscular type (apex, inferior or anterior wall): cardiomyocites; fibrous type (apex or subvalvular area): fibrotic tissue | Endo- myocardium |
